CUE SHEETS
Cue Sheets are used to keep track of
all music used in television, film, or radio productions. It is a
legal requirement and costs you nothing to submit a cue sheet. As
the primary means by which performing rights organizations track
the use of music in films and TV, cue sheets make it possible for
composers and publishers to be compensated for their work. Cue sheets
are only needed when the music will be broadcast on television, radio,
or appear in a film release as specified in your Music License Agreement.

CUE SHEET INSTRUCTIONS:

To submit a cue sheet for your production,
first open the Trakwerx Cue Sheet.
This is an easy-to-use online .pdf which you may fill out while
it's open on your monitor by simply clicking inside of the first
field and typing in the applicable information. You may then use
the tab key to proceed through each subsequent field.
NOTE: If you are on a Macintosh, you may need follow these instructions to access the fill-in fields in Adobe Acrobat Reader:
a. Open the Adobe Acrobat Reader.
b. Go to the Adobe Acrobat pull down menu at the top left of the screen.
c. Open preferences.
d. Click on "forms".
e. In the forms dialogue box there is an "auto-complete" pull down menu.
There are three choices: off, basic and advanced. Click on advanced. This will allow you to fill in and save data.

Once you've completed the form, click
your browser's print button to print and sign the form.*

Fax the form to Trakwerx at (818) 991-1944.

Submit a copy of the form to the
broadcaster (TV or radio station).

Submit a copy of the form to your
local Performing Rights Society. (See list below.)
*If you do not have a printer attached
to your computer, you may download a zipped copy of the Trakwerx
Cue Sheet for easy transport to a printer-equipped computer,
or simply email info@trakwerx.com
and request that a copy be mailed to you. Please include your full
mailing address.
